The following fields are NOT editable through the ESP Information form:

1. First, Middle and Last Name
2. Date-of-Birth
3. Gender

For security and fraud protection reasons, these fields can only be changed by contacting MyRapidMD Support.

To change these fields:

1. Send an email to support@myrapidmd.com with the subject "ESP Data Change Request"
2. Include in the body of your email (to verify your identity):
   
   1. Email address/username on record
   2. Member ID number
   3. Date-of-birth as it appears in your ESP
3. Specify the fieldname(s) you are requesting to be changed and BOTH the old and new values.
4. We will send you a confirmation email when the change has been made.
5. Send your ESP to your phone and download/install your updated ESP
